Insurance Information

Hereford Physical Therapy & Sports Medicine actively participates in many health plans and networks. These plans change on a regular basis. If you do not see your insurance plan listed, please call our office for further assistance.

We are required by our contracts with your insurance providers to collect all applicable co-payments, deductibles, and co-insurance at the time of service.

We offer to submit your insurance claims for you. However, in order to do this, we need to have all current insurance information, along with your signed authorization.

We will be happy to discuss payment options if necessary. Please note that you are ultimately responsible for payment of your account.
